Flagstar Home Health Services brings today’s modern medical technology to the patient’s home. Our team of dedicated professionals promotes “healing in the home” by allowing patients maximum freedom and independence in their place of residence.

Our Specialty Services Include:

Nursing

Our Home Care Nurses are committed to providing comprehensive, high quality care. They are highly skilled in patient assessment, planning, implementing and evaluating outcomes of multi-disciplinary care. They communicate with patients, families and other professionals within the health community. Our nursing staff includes both wound certified nurses and psych nurses to help with all stages of Alzheimer’s and dementia.

Rehabilitation

Our Rehabilitation Department is staffed with highly skilled physical therapists, occupational therapists, and speech/language pathologists. Our therapists communicate regularly with the patient’s physician. They will perform evaluations, plan treatment programs, educate, and utilize appropriate modalities to care throughout the healing process.

Diabetes/Nutrition Services

With proper education from our staff, diabetes can be managed effectively in the patient’s home environment. Nutrition plans are individualized for each patient by an on-staff dietitian. Special diets are created and will incorporate foods enjoyed by the patient. Additionally, we counsel patients on menu planning and management of their nutritional needs.

Social Services

Our medical social workers assist to resolve social, emotional, and financial problems that might hinder the patient’s treatment or expected rehabilitation potential. They have routine conferences with home care staff and family members. They can also help evaluate relocation of residence and assist with placement into senior apartments, assisted living communities, and nursing homes.
